Provide a summary of the project or program:
The Webster Council on Aging provides services to the rural isolated elderly of Webster Parish. The home-delivered meals programs reaches out to these seniors who live in our most isolated communities. The Webster Council on Aging is the point of entry into these services. Once an Outreach request is received contact is made with the prospective client and time for the intake assessment is arranged. An assessment provides a true picture of the senior's living conditions and any support needed. Emphasis is placed on frail, isolated home-bound elderly scoring highest on the assessment. Targeting the physically and mentality impaired and most isolated provides a challenge when skyrocketing cost of food, gas and supplies are increasing daily. The goal of the Webster Council on Aging is to prevent or prolong institutionalization therefore relieving the state of the burden of the cost of the facility based care.

What is the budget relative to the project for which funding is requested?:

Does your organization have any outstanding audit issues or findings?  No

If 'Yes' is your organization working with the appropriate governmental agencies to resolve those issues or findings?

What is the entity's public purpose, sought to be achieved through the use of state monies?
To help meet the nutritional needs of the isolated elderly in the rural parts of the parish. This will improve the quality of life of that older person; to prolong premature institutionalization. The elderly in these parts lack attention from family and most often depend on the meals driver for contact with the outside community.

What are the goals and objectives for achieving such purpose?
This money would relieve the home-delivered meals waiting list; to keep seniors where they are the happiest; provide for their nutritional needs. It is far cheaper to provide these services in the home than it is to see that elderly person placed in a nursing home setting.

What is the proposed length of time estimated by the entity to accomplish the purpose?
           one year.
