Provide a summary of the project or program:
Bayou Land Families Helping Families (BLFHF) will continue to serve Louisiana's Region 3 as its family-directed resource center for individuals with disabilities and their family members. We will provide assistance and support in navigating the very complex disability system. This program will provide new parents of children with a disability diagnosis with valuable resources at the onset of the diagnosis. BLFHF will work with hospitals within our region to provide information to families when a child is born with a disability.
Because it can be difficult for individuals to travel to our office to receive assistance, this program will also provide the staff at BLFHF the means to travel to a public location that is convenient for the family (library, school, etc.) in order to provide technical assistance and support in various areas of disability services. Some assistance provided may include information and/or referral about waivers, early childhood development, transition services, home- and community-based services, and family supports to name a few.
BLFHF also provides education to families and professionals on various disability-related topics. The BLFHF center has a training room that is equipped to provide education in-person, online, or hybrid. The delivery of this education can also be through peer-to-peer support or parent support groups.

Our mission is to provide information and referral, education and training, and peer-to-peer support to individuals with disabilities or special needs and their families. Our vision is to strengthen, support, and empower individuals with disabilities or special needs and their families. Bayou Land Families Helping Families is directed and staffed by parents and family members of individuals with disabilities and professionals with experience in disability services. It is this common experience that enables BLFHF staff to answer questions and provide technical assistance to individuals with disabilities, their families, and the professionals who serve them.
What are the goals and objectives for achieving such purpose?
Oftentimes, families turn to us when they don't know where else to turn. By the time they reach us, many are already struggling physically, financially, and/or emotionally. Our goal is to provide resources to families of individuals with any disabilities at the onset of the disability, whether it be at birth or in adulthood.

(d) The nature of the contract, including a description of the goods or services provided or to be provided pursuant to the contract:
The LA DDC contract establishes that BLFHF provide information and referral, education and training, peer-to-peer support, and systems advocacy to individuals with developmental disabilities and their families. The goal is to support inclusion, self-determination, and access to services through one-on-one support, community outreach, and participation in legislative advocacy efforts.
The Louisiana Department of Education contract supports Bayou Land's efforts to improve family engagement and educational outcomes for students with disabilities. Services include Individualized Education Plan (IEP) support, transition planning, parent training, and technical assistance to families navigating special education services in public schools.
Through the contract with the Bureau of Family Health, Bayou Land Families Helping Families provides care coordination, resource navigation, and family support services for families of children and youth with special health care needs. This includes connecting families to medical homes, early intervention programs, and social services to improve health and developmental outcomes.
