§450.8. Continuous glucose monitors; Medicaid coverage
A. Except as provided in Subsection B of this Section, the Louisiana Medicaid
program shall provide coverage for a continuous glucose monitor for the treatment of an
enrollee who has one of the following conditions:
(1) Any type of diabetes with the use of insulin more than two times daily or
evidence of level 2 or level 3 hypoglycemia.
(2) Glycogen storage disease type 1a.
B. To qualify for coverage pursuant to Subsection A of this Section, the enrollee
shall attend regular follow-up visits with a healthcare provider at a minimum of every six
months to assess the ongoing benefit of the enrollee's use of a continuous glucose monitor.
C.(1) The Louisiana Department of Health shall implement the provisions of this
Section using the most cost-effective solution available to the department.
(2) The secretary of the department shall submit to the Centers for Medicare and
Medicaid Services all state plan amendments, promulgate all rules and regulations in
accordance with the Administrative Procedure Act, and take any other actions necessary to
implement the provisions of this Section.
D. For the purposes of this Section, "continuous glucose monitor" means an
instrument or device, including repair and replacement parts, that is designed and offered for
the purpose of measuring glucose levels at set intervals by means of a small electrode placed
under the skin and held in place by an adhesive.
