§444. Pool account leave request

            Employees of a state agency requesting leave from the pool leave account under the provisions of this Part shall submit a written leave request to the employing agency with the following information included:

            (1) The name, employing agency, position, title, and classification of the employee.

            (2) A brief description of the nature, severity, and anticipated duration of the personal emergency.
